چکیده
Abstract Background and purpose Coronary microvascular dysfunction (CMD) has been proposed as a key driver in the etiopathogenesis of Takotsubo syndrome (TTS), likely related to an “adrenergic storm” upon susceptible circulation. The aim our manuscript was assess quantify CMD patients with TTS through angiography-derived index microcirculation (IMR) evaluate its correlation clinical presentation. Methods angiograms 41 consecutive were retrospectively offline analyzed derive angiography-based indices CMD. Three recently developed (NH-IMRangio, AngioIMR A-IMR) calculated compared based on Quantitative Flow Reserve (QFR) analysis. defined IMRangio ≥25 units. between presentation outcomes then assessed. Results Median age 76 years, 85.7% women mean left ventricular ejection fraction (LVEF) at first echocardiogram 41.2%. Angiography-derived IMR higher Left Anterior Descending artery (LAD) than Circumflex (LCX) Right (RCA) either NH-IMRangio (52.7 vs 35.3 41.4), (47.2 31.8 37.3) or A-IMR 36.1 41.8). All presented least one territory each formula. LAD significantly presenting LVEF impairment (≤40%) those preserved global function (NH-IMRangio: 59.3 46.3, p. value=0.030; AngioIMR: 52.9 41.4, p-value=0.037; A-IMR: 59.2 p-value=0.035). Conclusion CMD, assessed IMR, is common finding it inversely correlated LV function. available formulas have substantial superimposable diagnostic performance assessing coronary Funding Acknowledgement Type funding sources: None.

Microvascular Dysfunction in Takotsubo Cardiomyopathy
Takotsubo cardiomyopathy (TC), also called stress cardiomyopathy, apical ballooning syndrome or broken heart syndrome, is a syndrome characterized by transient regional systolic dysfunction of the left ventricle (LV), mimicking myocardial infarction (MI), but in the absence of evidence of angiographic significant obstructive coronary artery disease (CAD) or acute plaque rupture. In most cases o...
متن کاملThermodilution-derived coronary microvascular resistance and flow reserve in patients with cardiac syndrome X.
BACKGROUND Although increased coronary microvascular resistance (CMR), resulting in coronary microvascular dysfunction, is speculated to be responsible for myocardial ischemia in patients with cardiac syndrome X (CSX), it has never been directly demonstrated, and the correlation between CMR and severity of myocardial ischemia has not been elucidated in this setting. This study aimed to ascertai...
متن کاملTakotsubo Syndrome
T akotsubo syndrome (TTS) is an acute heart failure syndrome that predominantly affects post-menopausal women. It is characterized by substantial morbidity and mortality, which equal those of acute coronary syndromes (ACS) and are still underappreciated (1). TTS and ACS share many clinical features on admission, and an estimated 1% to 2% of all patients with suspected ACS are finally diagnosed ...
متن کاملInfluence of percutaneous coronary intervention on coronary microvascular resistance index.
BACKGROUND Coronary microvascular resistance during maximal hyperemia is generally assumed to be unaffected by percutaneous coronary interventions (PCIs). We assessed a velocity-based index of hyperemic microvascular resistance (h-MR(v)) by using prototypes of a novel, dual-sensor (Doppler velocity and pressure)-equipped guidewire before and after PCI to test this hypothesis. METHODS AND RESU...
متن کاملApical ballooning syndrome (Takotsubo Syndrome): case report
INTRODUCTION The apical ballooning syndrome (ABS) is a single reversible cardiomyopathy often triggered by a stressful event. We aimed to present a case report regarding this disorder. CASE PRESENTATION Here we present the case of a 77-year-old female hypertensive patient, sedentary and non-smoker, diagnosed with apical ballooning syndrome. We describe the clinical signs and symptoms, changes...
